Responsibilities:
- Work in partnership with the Case Manager to provide skilled speech therapy services to assist patients with breathing, speaking, and swallowing functions.
- Evaluate and treat speech, language, cognitive, and fluency disorders that impact communication.
- Serve as an educator, patient advocate, and integral member of the interdisciplinary care team.
Requirements:
- Current, unrestricted CA Speech Therapist License
- Current BLS/CPR Certification
- Valid CA Driver’s License
- Understanding of individual development and human behavior as it relates to illness/injury
- Familiarity with home health care regulations (preferred)
- Experience with home health care and high-acuity patients (preferred)
- Strong case management skills (preferred)
- Self-directed, empathetic, critical thinker with outstanding interpersonal and leadership skills

Salary Range: $110,000 - $125,000 per year
Starting pay is commensurate with relevant experience above the minimum requirements. Bonus incentive plans, comprehensive health benefits, and 401k matching up to 4% are available.
