What are the responsibilities and job description for the LPN/RN - FT PM Shift position at Fond du Lac County?
Under the general direction of the Director of Nursing, ensures quality nursing care to residents on an assigned unit. Directs the planning of individual nursing care plans, directs staff involved in patient care, and communicates effectively with other disciplines involved in patient care. Participates on nursing committees and the management team, directs the operation of the units, and performs related duties as required.
· Directs the planning of individual nursing care plans. Coordinates the staffing of patients; develops, implements, and evaluates specific care plans, and assesses changes in resident conditions and determines immediate and follow-up care.
· Works with the attending physician in keeping monthly records, progress notes, patient visits, and other related materials up-to-date. Documents and follows physicians' orders.
· Provides complete and accurate reports to other shifts and conducts precise, accurate, and complete charting.
· Directs staff involved in patient care. Conducts direct patient care as indicated and identifies duties to be delegated. Provides guidance and teaching to staff, and presents in-service to staff. Supervises nursing procedures and performance, investigates personnel problems, and completes staff performance appraisals.
· Ensures adequate staff coverage for units. Reviews and revises nightly care sheets and responsibility lists. Assists lab personnel.
· Communicates effectively with other disciplines involved in patient care. Cooperates with the attending physicians and relates specific needs to appropriate disciplines.
· Participates on nursing committees and the management team. Completes quality assurance studies.
· Directs the operation of the units. Complies with and enforces the Patient Care Manual for each unit, maintains and orders unit supplies, and completes inventory checks. Regularly checks equipment to maintain safety.
· Ensures a safe, therapeutic environment which protects patients' rights and safety for residents and staff, and investigates accidents occurring while in charge of the unit.
· Administers staff immunizations and performs related duties as required.College degree in nursing, one year of medical/surgical or geriatric nursing experience, and a Wisconsin Registered Nurse license.
Desirable Knowledge and Abilities
Knowledge of geriatric, DD and CMI needs and concerns, federal and state regulations, current nursing practice and residents' rights. Ability to make accurate assessments, provide crisis intervention, communicate effectively orally and in writing, and interact effectively with patients, their families and the general public.
LPN Rate Range: $27.12-$34.51
RN Rate Range: $37.72-$47.99
