Indian Child Welfare Social Worker/Children's Mental Health Case Manager

Responsibilities: Accept referrals and assess unmet needs of Indian families.
Referrals are accepted from other providers and the Social Services Coordinator, most often following an introductory discussion between the client and the referring person or an order for the court.
Develop service plans to address unmet needs of family.
Following and assessment meeting with the client/family (and other providers when appropriate) a reasonable service plan is developed.
To avoid duplication of services and because Minnesota is a PL 280 state where the counties are ultimately responsible for client care, either the county or the reservation social worker will be the primary case manager, with both agencies being informed of proposal plans.
The service plan will include reasonable goals for individuals within the family (household) and the family as a unit.
Serve as the Children’s Mental Health Case Manager when appropriate.
Goals may include all types of family activity that will contribute to healthier behaviors,
Other providers (FDL and county persons) may be included as a team effort.
While action is taking place to achieve the goals of the service plan, the reservation primary case manager will update the court and other agency of activity.
When the family behavior has reached the healthier level in day-to-day activity, and the family worker time in the home has decreased or has ceased, the case can then be transferred to secondary status or to the county for primary case management and monitoring only be the reservation social worker.
Serve as primary case management for Indian families.
Cases will be prioritized to allow family worker time for the complex situations and those families who are willing to put forth a cooperative effort.
Serve as secondary (informed monitor only) case manager as needed.
Provide individual family counseling and assess need for formal therapy versus in-home family type counseling.
Make appropriate referrals when necessary.
Schedule in-home time with the family (and team member) when appropriate and as often as necessary.
Refer clients to other service providers as appropriate.
Be knowledgeable of in-hours and area resources.
Use appropriate referral avenues.
Follow-up the referral with a team meeting when necessary.
Allow the proper closure when possible.
Advocate for client and serve as liaison with other providers.
Accompany clients to family court when necessary.
Assist clients in initial meetings with other providers.
Assist with the training of Fond du Lac foster homes.
Consult with other service providers on individual and family needs.
Establish and maintain a networking system between FDL, CAIR and other reservations social workers.
Assist in the planning, developing and presentation of recreational/educational events in the community.
Submit appropriate articles to newspaper for Indian readership.
Complete monthly, quarterly, and annual reports promptly.
Displays a responsive and professional manner in promptly responding to all requests, complaints, and problems.
Recognizes that each employee is a representative of the Fond du Lac Band of Lake Superior Chippewa and is responsible for demonstrating courtesy, respect, and sensitivity to the needs of all others, including visitors and co-workers.
Represents the Fond du Lac Band of Lake Superior Chippewa and the department in a positive and professional manner in the community.
Required to maintain proper attendance including reporting to work on time in accordance with applicable policies.
Maintains a clean and organized work area.

• Due to changes and modifications in the job from time to time, employees are required to be flexible and assume other responsibilities assigned by management as deemed necessary.

Qualifications: Valid U.S. Class D Driver’s License with Fond du Lac Insurability is required.
Bachelor’s Degree in Social Work or related field is required.
Two years field experience in professional level job and case management experience is preferred.
Master’s degree is preferred.
Ability to communicate effectively orally and in writing is required.
Ability to work independently and establish priorities is required.
Attention to detail and accuracy is required.
Ability to establish professional and harmonious working relationships on all projects and with all parties involved.
Subject to testing in accordance with the Fond du Lac Reservation’s Personnel Policies.
Subject to pre-employment and annual background checks.
Subject to immunization review and submitted proof of vaccinations.
Travel is required.
