REPORTS TO: Site Director
PROGRAM SUPERVISOR: Regional Director for Student Services
QUALIFICATIONS:
- College degree required.
- Valid Georgia School Social Worker certificate If the Social Worker does not have a Social Work certificate, they will collaborate with a certified Social Worker from a partner site on certain job tasks.
- Experience with high school students preferred.
- Ability to take direction and perform tasks independently.
DUTIES:
Student Services
- Attend and participate in monthly Student Services Meetings as part of the site student services team.
- Regularly communicate with Mentors and Mentor Facilitator around student needs.
- Collaborate with other student services staff (Career Specialist, Counselor, Mentor Facilitator, Student Services Manager) to plan and implement activities.
Attendance
- Assist in Monitoring weekly student attendance. Develop and implement attendance intervention; work to solve transportation issues.
- Work closely with Counselor, Site Administrators, Career Specialist or other staff to develop and support on-going Drop-Out Prevention and Drop-Out Recovery programs.
Direct Services/Student Support
- Conduct home visits as needed.
- Based on referral needs, counsel with students and parents; help families cope with economic struggles such as financial needs, housing, applying for Medicare/Medicaid, childcare, etc.
- Provide referrals for students or families needing medical treatment, mental health services, legal services, and other community services.
- Participate in the development of Individual Education Plans (IEP) and/or MTSS process as required.
- Follow up as appropriate with all referrals and enter casenotes.
- Work with the Regional Student Services Coordinator-Social Workers/McKinney Vento/Foster Care about services for students experiencing homelessness and students served through foster care.
- Serve as the site point-of-contact for students with severe health or mental health needs who may need services through HEAL (Hospital or Home Environment Access to Learning) and facilitate all paperwork and meetings.
- Assist with site referrals to the diaper bank, food bank or other community based resources.
Internal Support and Professional Development
- Train all staff on how to use the SWARM database to make referrals for services.
- Maintain appropriate school records and provide written reports and communication as appropriate.
- Read Foothills emails in a timely manner.
- Respond to surveys and reports by deadlines.
- Provide Site Director with appropriate data as requested.
- Participate in professional development activities to improve knowledge and skills.
- Perform other duties as assigned.
