The Early Intervention Service Coordinator is responsible for planning, coordinating, and monitoring of services and supports provided to families of infants and toddlers eligible for Federally funded Part C services and State funded Early Intervention services. The EI Service Coordinator shall have an assigned caseload of individuals for whom he/she shall be responsible to advocate. The EI Service Coordinator will facilitate initial and ongoing Individualized Family Service Plan (IFSP) meetings, complete family assessment, support the family to ensure services are being delivered in accordance with the IFSP, monitor the provision of services, supports, and recommendations identified in the IFSP, assist in obtaining identified services and supports, maintain a master record and all necessary documentation, serve as the primary contact for obtaining and disseminating information, and facilitate and mediate conflict resolution. Inherent in all these functions shall be activities supporting families Rights and Procedural Safeguards under Part C, support, empowerment, and advocacy.

Requirements/Responsibilities:
- Bachelor’s degree required; or five years of experience in the field of developmental disabilities.
- Must be able to work some flexible hours and have own transportation.
- Must be sensitive to infant/toddler delays/disabilities and their needs.
- Must demonstrate respect for cultural and ability diversity.
- Must display a high degree of responsibility, initiative, interpersonal skills, and communicative abilities.
- Must have the ability to perform complex or varied tasks.
- Contact all assigned incoming families.
- Demonstrate ability to handle multiple assignments and competing priorities. Ability to multi-task while remaining highly organized in a fast-paced work environment required.
- Assist the family to identify and secure services and supports as outlined in the IFSP.
- Assist in maintaining the person's eligibility for services and benefits as necessary and appropriate.
- Assure all follow-ups completed and documented.
- Driving is a required function of this job.
-1 full time position available at $23.83-$26.41/hr. DOE/DOQ,
