What are the responsibilities and job description for the Social Services Director position at Fordsville Nursing and Rehabilitation?
Position Purpose: Responsible Collaborates with nursing and nursing management, and administration to promote and protect resident rights, dignity, independence and psychological well being. Maintains good working relationships with all employees, meeting with admin and nursing staff to collaborate and plans comprehensive psycho social assessment is completed for each resident that identifies social, emotional, psychological needs and strengths. Educates residents and families regarding their rights and responsibilities, health care decision making/advanced directives and effective problem solving.
ESSENTIAL FUNCTIONS OF POSITION:
- Plans and directs a comprehensive social service program which provides support services for residents and families.
- Responsible for coordinating all admissions and discharges to ensure facility census is kept at its maximum capacity.
- Participates in an interdisciplinary team approach to better serve the needs of the facility population.
- Maintains written documentation in the customer medical record per facility policy and state and federal guidelines.
- acts as a liaison/advocate between residents, families, outside agencies, and the administrator to ensure that the resident rights are maintained.
- Facilitates discharge planning including the development of an organized discharge plan.
- Perform additional duties as assigned.
Qualifications / Requirements:
- Preferred Bachelors degree in related field.
- Minimum of two years experience in a supervisory capacity in a Long Term Care or equivalent.
