Key Responsibilities:
- Resident Support: Assess the social, emotional, and psychological needs of residents, providing personalized care and support to enhance their well-being.
- Care Plan Development: Collaborate with the interdisciplinary care team to develop and implement individualized care plans, ensuring residents' social services needs are addressed.
- Family & Resident Advocacy: Act as a liaison between residents, families, and the facility to facilitate communication, support family involvement, and advocate for residents’ needs.
- Discharge Planning: Coordinate discharge planning for residents transitioning from the facility, ensuring a smooth process by assessing post-discharge needs and making referrals to appropriate community resources.
- Crisis Intervention: Respond to resident or family crises, providing emotional support and assistance with problem-solving during difficult situations.
- Community Resource Coordination: Identify and connect residents and families to community resources and services, including ancillary services, support groups, and other social services.
- Documentation & Compliance: Maintain accurate records of social services assessments, interventions, and follow-ups, ensuring compliance with facility policies and regulatory requirements.
- Team Collaboration: Work closely with other healthcare professionals—including nurses, therapists, and physicians—to provide holistic care and ensure all aspects of residents' well-being are addressed.
- Resident Rights Advocacy: Promote and protect residents’ rights, ensuring they are treated with dignity and respect, and provide information on their rights and responsibilities within the facility.
Qualifications & Skills:
- Bachelor’s degree in Social Work, Psychology, Sociology, or a related field is preferred but not required.
- Experience in healthcare or long-term care settings is preferred, particularly in skilled nursing or rehabilitation.
- Strong communication and interpersonal skills to effectively work with residents, families, and interdisciplinary teams.
- Ability to handle sensitive issues with empathy, professionalism, and confidentiality.
- Knowledge of social services resources and community programs.
- Proficiency in documentation and record-keeping in accordance with facility policies and regulations.
- Problem-solving and crisis management skills to handle a variety of challenging situations effectively.
