FT Hospice Social Worker

About the Role:

We are seeking a compassionate and dedicated Hospice Social Worker to join our interdisciplinary team. The Hospice Social Worker provides emotional support, counseling, and resource coordination to patients and families facing end-of-life challenges. This role is essential in helping individuals navigate complex medical, emotional, and practical needs, ensuring the best possible quality of life for patients and their loved ones.

Minimum Qualifications:

• Bachelor's or Master's degree in Social Work or a related field
• Current state licensure as a Social Worker
• Experience providing crisis intervention, case management, and psychotherapy in a healthcare setting
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team

Preferred Qualifications:

• Experience working with diverse hospice populations
• Experience with mental health counseling
• Bilingual in English and Spanish

Responsibilities:

• Patient and Family Support
  • Assess the psychological, emotional, and financial needs of patients and families.
  • Provide counseling and support to address grief, loss, and coping strategies.
  • Facilitate effective communication between patients, families, and the care team
• Resource Coordination:
  • Advocate for patient and family needs with external agencies as necessary.
  • Assist families accessing community resources as needed.
• Care Planning:
  • Participate in interdisciplinary team meetings to develop and update care plans.
  • Collaborate with healthcare provides, spiritual counselors, and other professionals to ensure holistic care.
  • Ensure compliance with federal, state, and organizational regulations and standards.
• Documentation:
  • Maintain accurate and timely documentation of patient interactions, assessments, and interventions.
  • Ensure documentation meets legal, regulatory, and organizational requirements.
• Education and Advocacy:
  • Provide education to patients and families about end-of-life care options and decision-making
  • Advocate for patient preferences and advance directives.
  • Facilitate discursions on hospice philosophy and services

Physical and Work Requirements:

• Ability to travel to patient homes, care facilities, and other locations as needed.
• Flexibility to work occasional evenings or weekends based on patient and family needs.

Skills:

As a Social Worker, you will use your crisis intervention, case management, and psychotherapy skills on a daily basis to help improve the overall well-being of our patients and their families. You will also need strong communication and interpersonal skills to collaborate with other healthcare professionals and maintain accurate patient records. Additionally, experience working with diverse populations and mental health counseling will be beneficial in this role. Bilingual skills in English and Spanish are also preferred.
