Job Summary:
Responsible for overseeing and ensuring quality care management that is in accordance with the guidelines and regulations set forth by the DOH and Forestdale/ CCF. The Health Home Care Management program assists to ensure all clients have access to quality medical and mental health care.
Major Responsibilities/Activities:
- Supervise a unit of Care Managers with a mixed acuity case load ranging from 20-40 clients each.
- Conduct weekly/biweekly supervision with each Care Manager to review case details as it relates to case needs, overall well-being and progress of each family.
- Participate in Interdisciplinary meetings, Case Conferences, CCF/DOH required meetings and other staff meetings.
- Ensure and track that staff are attending all trainings as required and remain updated with any needed certifications (i.e.; CANS-NY)
- Facilitate team meetings to support and guide their team as needed.
- Monitor timely submission of all documentation as required by DOH in GSI.
- Review all documentation to ensure services provisions are in compliance with DOH and CCF regulations.
- Oversee the identification of eligible Health Home youth referred for program enrollment and consent process.
- Coordinate and ensure execution of plans for audit preparation. Participate in audits as needed.
- Carry a small caseload depending on program needs
- Other duties as required.
Minimum Requirements:
- Master’s Degree in Social Work, Nursing, Public Health, or related field, plus appropriate licensure
- Knowledge of best practice working with children and families in health, preventive and wellness care
- Bilingual a plus
- Working knowledge of child welfare, developmental disabilities, mental health and/or primary healthcare fields
- 2 years’ experience managing teams and/or providing care management
- Experience using Excel and electronic health care systems to review and manage data preferred
- Experience providing service coordination and information, linkages, and referrals for
- Community-based services
In addition the candidate must demonstrate the following competencies:

- Ability to travel to community-based agency offices and clients’ homes
- Proven decisiveness, motivational, and developmental skills; management abilities, including meeting deadlines, ensuring compliance with agency policies and procedures, and overseeing complete and timely maintenance of agency records, in accordance with contractual requirements
Salary Range:
- Masters: $65,000 to $73,000
- Masters / Language: $67,000 to $75,000
- Masters / License: $70,000 to $78,000
- Masters / License / Language: $72,000 to $80,000
