MEDICATION HISTORY PHARMACY TECH I

Job Summary : # Obtains medication histories for new patients and documents them in the electronic medical record in a timely manner. Medication history will be obtained from patient, patient#s family, pharmacy, physician office, or long term care facility.# Employee must be diligent to fully explore the medication history. Communicates with pharmacist appropriately including asking questions, and referring for additional interview or intervention. Uses good clinical judgement#to update the medication history to the best possible list. Demonstrates knowledge and skills to appropriately communicate and interact with employees, patients, families and visitors of all age groups while being sensitive to their cultural and religious beliefs. Included within this role assignment is the responsibility to be a positive role model for all peers always reflecting the WE CARE philosophy. Demonstrates ability to perform technician duties as deemed appropriate by management and supervising pharmacist. Demonstrates basic understanding of the 340b program.
