What are the responsibilities and job description for the Patient Care Coordinator- Hospice position at Forrest Health?
RN is a member of the patient care team within the Hospice Department. The Patient Care Coordinator is accountable to the Patient Care Manager or Director. The Patient Care Coordinator is responsible for education of Nurses and Nurse Aides each month. The PCC is responsible for Orientation of staff in compliance with CMS. The Educator- PCC shall implement the nursing process in an organized, systematic manner to include assessment, planning, intervention, scheduling, and evaluation in collaboration with the Hospice IDT. . Performs other job duties as assigned. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his/her age specific needs, and to provide the care needed. The PCC-Educator is a hospital liaison and makes contacts and builds relationships with the hospital and follows up on referrals. Responsible for Chart Audits, regulation for home hospice and IP. PCC leads the yearly competencies and coordinates with the Program coordinator to maintain all staff are in compliance.
Performance Expectations:
- Demonstrate ability to assist nurses with caring for patients with complicated wound care and dressings.
- Demonstrate ability to oversee medication administration in the facility by peripheral and IV pump administration.
- Demonstrate ability to enter all patients charting in EMR according to policy.
Qualifications:
Education/Skills
Graduate of a recognized non-online RN program required.
Work Experience:
Minimum of two years as a staff RN in an acute setting required, with six months being at Forrest General Hospital preferred.
Certification/Licensure-DUE UPON HIRE
- Licensed RN able to practice within the State of MS

- Basic Life Support
