What are the responsibilities and job description for the PT OUTCMS COORD position at Forrest Health?
The Patient Outcomes Coordinator performs under the direct supervision of the Director of Quality Management Services and is accountable for providing leadership in planning, implementing, evaluating, and integrating performance measurement and improvement activities. Facilitates the improvement of patient care processes and outcomes using evidence-based approaches with the input and clinical experience of those who will be providing the care. Performs data abstraction and analysis for quality measures with responsibility for leading improvements. Responsible for patient outcome information management functions including design, analysis, and interpretation. Collaborates with administrative and clinical leaders to resolve outcome issues. Demonstrates knowledge and skills to appropriately communicate and interact with the patients, families, and visitors of all age groups while being sensitive to their cultural and religious beliefs. Performs other duties assigned by the QM Director.

- Demonstrates ability to review and abstract pertinent clinical information from the electronic medical record.
- Demonstrates ability to analyze data and identify opportunities for improvement.
- Demonstrates ability to effectively communicate with managers, staff and physicians.
- Demonstrates ability to accurately enter pertinent clinical information into third party vendor data collection tools.
Qualifications:
Education/Skills
Graduate of a recognized non-online RN program program required. Bachelor degree preferred with statistical and basic computer knowledge.
Work Experience:
A minimum of five years of diverse clinical nursing practice preferred.
Certification/Licensure-DUE UPON HIRE
- Current License with the MS State Board of Nursing
