The Community Outreach Liaison serves as a vital connector between Forsyth County Health and Human Services (HHS) and the community, leading efforts to strengthen engagement and expand awareness of essential services. Working across all four HHS divisions—Public Health, Social Services, Behavioral Health Services, and the Bridges to Hope Family Justice Center—the Liaison plays a key role in building relationships with stakeholders, coordinating educational events, and representing the department at community gatherings and initiatives.
This position may require working some evenings and weekends.A successful candidate should have the following knowledge, skills and abilities:
Strong organizational and communication skills
The ability to work independently in a remote setting while managing multiple responsibilities
Proficiency in Microsoft or similar software is a plus
Ability to travel locally for site visits and community eventsGraduation from a four-year college or university with a major in health education.
A four-year degree outside of the relevant academic field plus additional years of relevant experience may also be considered.
A higher education level may be considered as a substitution for all or part of the experience.
Valid driver's license required.

- Organize and participate in quality-focused community events, such as educational workshops, healthcare expos, and support groups.
- Serve as an educational resource for healthcare providers, schools, families, and other community stakeholders.
- Partner with community organizations to distribute educational materials and increase the visibility of HHS programs and services.
Referral Management and Stakeholder Networking
- Conduct scheduled site visits per day to referral sources such as healthcare providers, schools, community organizations and local businesses.
- Maintain detailed records of visits in Microsoft, including key discussion points, follow-ups, and next steps.
- Foster strong, ongoing relationships with stakeholders to build trust and ensure service transparency.
- Engage in meaningful relationship-building conversations to understand referral needs and service improvements.
Event Planning and Execution
- Plan and execute educational events, prioritizing quality outreach over quantity.
- Coordinate with community partners to create impactful, information-driven events.
- Track event performance and identify key takeaways to refine future event strategies.
Reporting, Follow-Up, and Post-Communication
- Maintain a structured daily schedule, logging referrals, tracking outreach progress, and preparing follow-up communications.
- Submit detailed weekly reports summarizing visits, event outcomes, and referral generation results.
- Ensure all inquiries and referral requests are addressed within 24-48 hours.
- Provide consistent updates to leadership, including successes, challenges, and suggested action steps.
