Job Summary:
We are seeking a highly skilled and compassionate Case Manager II to join our social services department. As a key member of our team, you will be responsible for providing critical support to veterans, their spouses, and dependents. Your primary duties will include case coordination, psychosocial assessments, and resource planning to ensure that our clients receive the highest level of care and support.
Key Responsibilities:
- Conduct comprehensive client and family needs assessments to identify barriers and develop effective service plans.
- Provide brief counseling and crisis intervention services to help individuals and families overcome challenges and achieve self-sufficiency.
- Develop and implement individualized service plans in collaboration with clients, healthcare providers, and other stakeholders.
- Connect clients with community resources and provide education on available services to promote independence and well-being.
- Participate in outreach activities and collaborate with internal and external stakeholders to reach underserved populations.
- Maintain accurate records and reports in accordance with professional standards and policies.
Requirements:
- Bachelor's degree in Social Work, Counseling, or a related field, or equivalent experience.
- Minimum 2 years of experience working with vulnerable populations in social services, preferably with veterans.
- Strong written and verbal communication skills, with ability to work effectively with diverse groups.
- Ability to travel and participate in outreach events as needed.
- Familiarity with case management software and grant-funded programming preferred.
