Job Summary
The Clinical Care Coordinator Lead plays a pivotal role in ensuring seamless care delivery to patients with complex needs. This individual will develop and implement comprehensive care plans, coordinate inter-organizational efforts, and facilitate patient-centered care.
Key Responsibilities
- Develops and implements comprehensive care plans that cater to the unique needs of patients
- Coordinates with ambulatory clinics, inpatient services, and community agencies to ensure continuity of care
- Facilitates wrap-around meetings and team conferences to address patient needs
- Observes and assesses patient health status through physical examination, interviewing, and reviewing medical history
- Educates and counsels patients on disease management, prevention, and healthy lifestyle choices
Requirements
- Two years of experience as a Registered Nurse in a clinical setting
- Associates of Science degree in Nursing from an accredited program
- Valid and unrestricted Registered Nurse license in any U.S. state or territory
- American Heart Association certification in Basic Life Support
