- Provides care to assigned patients under the direction of a Public Health Nurse and department lead nurse/case manager in accordance with established policy. Provides basic care to patients with a variety of conditions, illnesses and/or injuries (i.e., apply and remove dressings, slings, braces, etc., apply heat and cold packs, assist patient in ambulation, transferring, positions, etc.).
- Monitors patients, reporting to and working under the direction of the Public Health Nurse and primary lead nurse/case manager. Shares in the responsibility for reporting changes in patients’ condition by observing the patient’s condition and reaction and by keeping the provider informed if conditions change. Responsible for obtaining and recording patient information as assigned per unit policy (i.e., vital signs, height, weight, temperature, respiratory rate, I&Os, etc.).
- Maintains a courteous and professional manner in all contact with patients, family and other customers. Assists in answering the phone and supplying routine information per established department guidelines.
- Shares responsibility for the maintenance of clinical records. Adheres to confidentiality polices and the Privacy Act.
- Inputs data into unit computer systems (e.g., ADT, immunization package, etc.). Use of Electronic Health Record (EHR) system to enter and retrieve data needed for patient care, i.e., lab test results, immunizations, health status, scheduling appointments, sending referrals, ensuring records have adequate forms.
- Educates and advises patients on specified medical issues within established parameters and documents as per hospital policy. Enhances patient understanding through interpretation when indicated.
- Practices Infection Control/Safety and environmental surveillance principles to protect self, patients, family members, visitors of all ages and other employees.
- Assists with collection and proper care of specimens (labeling, handling and delivery),
- Performs waived testing (e.g., glucometer testing) and records the results in compliance with regulations/standards. Complies with the laboratory requirements (CLIA, CMS, etc.) for quality control waived testing for accuracy and record keeping.
- Performs general maintenance duties including, but not limited to, cleaning, arranging and restocking supply cabinets, cleaning and sorting supplies, instruments and equipment, calling attention to deficiencies in supplies and equipment. Assists in inventory of unit equipment. Stocks exam/procedure room with proper instruments, supplies and equipment. Assumes initiative in reporting
- Assists in orientation of new personnel.
- Works under protocols of care established for the Hospital and its facilities, standards of care and practice, departmental policies and procedures and uses judgment in selecting the most appropriate application of the guidelines based on the patient’s condition and previous instructions. Unusual developments are referred immediately to the Public Health Nurse.
- Contacts patients and their families are to obtain information, teach, encourage, improve or change health habits. Contact with hospital and community healthcare providers is to exchange, provide and obtain information. Other contacts with outside agencies are to coordinate healthcare and other services. Contact with non-professional hospital/clinical staff is to maintain an environment of care and supplies within the department.
- Arranges and assists with making appointments for patients within the hospital facilities.
- Participates in the relevant committees and meetings regarding the Public Health Nursing Program/Division of Community Health.
- Participates in the Quality Management activities and data management responsibilities.
- Attends all mandatory and educational in-services relating to patient care.
- Makes patient home visits alone or with Public Health Nurse and/or other health professionals, explains illness, treatment and care being given.
- Interprets for the Public Health Nurse to individual clients, family members, groups, outreach clinics and other community settings about healthcare problems, needs, instructions, and provides health teaching and education.
- Provides resources for health care providers on the Navajo tradition/belief system, i.e. health concepts, cultural beliefs, and health practices.
- Elicits information, explains informed consent, delivers appointments and explains referrals concerning a variety of health related topics and/or disease processes.
- Takes vital signs, temperature, height, weight, blood pressure and elicits history of complaint if needed.
- Receives specimens such as sputum, urine, etc., taken from patients for delivery to hospital or clinic. As instructed by Public Health Nurse, handles and labels appropriately.
- Presents health education on a variety of topics to groups and/or individuals in homes, schools, businesses, chapters and other locations under the direction of the Public Health Nurse.
- Creates bulletin board displays, pamphlets, teaching materials, public service announcements, and other media for the department.
- Presents health education on a variety of topics. Sets up display and dismantle, inventorying supplies.
- Acts as a receptionist, registering patients, accessing records and directing flow of patients.
- Maintains records and reporting systems according to established hospital policies.
- Retrieves and files record utilizing EHR, records health data in medical records and researches records data.
- Schedules appointments for patients.
- Assists with inventory and storage or supplies.
- Perform other duties as assigned.
Experience:
