Educates patients within the Fort HealthCare outpatient Diabetes Self-Management Education and Support (DSMES) program. Collaborates with Fort HealthCare providers, staff, and community members to provide diabetes education to various age groups while following Fort HealthCare’s mission and vision.
Responsibilities of this position include:
- Assists in the planning, implementation, and evaluation of the diabetes self-management education and support (DSMES) program at Fort HealthCare.
- Provides DSMES education, using evidence-based practice guidelines, within the diabetes care team to improve patient outcomes.
- Understands and communicates to providers the reimbursement criteria related to DSMES program and encourages referrals as appropriate.
- Serves as a resource and liaison to the FHC’s School Nurse Program.
- Maintains current practice, using evidence-based guidelines in diabetes management and Population Health strategies by attending educational opportunities accepted by the American Diabetes Association (ADA) as well as any other pertinent organizations.
- Assists in maintaining the Electronic Medical Record (EMR) of persons receiving diabetes education.
- Maintains knowledge on the use of a variety of diabetes management equipment including insulin pumps, insulin pens, glucometers, continuous glucose monitors, etc.
- Serves as a resource person for FHC providers, Fort Medical Group (FMG) providers, clinical staff, and the community.
- Facilitates diabetes/glucometer education in clinical new employee orientation (CNEO).
Requirements for this position include:
- Associate degree required
- Bachelor, Masters, or Doctorate degree preferred
- Experience in patient education; 3 years' experience in a hospital setting required
- Computer skills in Word, Excel, and PowerPoint required
- Motivational interviewing techniques preferred
