REPORTS TO: Physician/Medical Director
DEFINTION/PURPOSE:
Under general supervision from the Medical Director, develops, plans and implements physical fitness activities and education with a special emphasis on diabetes prevention for children, youth, adults and seniors who are seen at Wassaja Memorial Health Center.
ESSENTIAL FUNCTIONS:

TASKS:
- Assesses the patient’s fitness level upon entry into any program activity. Administers fitness testing/screening devices to determine health risk; makes recommendations for and implements exercise prescription and programs. May refer patients to health or social services, as appropriate.
- Monitor’s individual patient’s progress; counseling patients regarding fitness and exercise and recording information in the patient’s file. Maintains appropriate patient records. Works with patients in adjusting exercise prescriptions and program to meet their needs. Reviews patient participation in programs to ensure appropriateness of scheduled activities; reviews participation to provide a diversity of accessible classes at all fitness levels.
- Organizes group exercise programs; serves as leader for group exercise programs. Designs and establishes choreography for exercise programs. Prepares class schedules to meet client needs and available staffing requirements. Adjusts schedules as needed to attain maximum participation of patients. Monitors presentations by instructors working under contract, as appropriate.
- Conducts participant surveys to determine satisfaction with programs and to assess on going needs. Makes necessary program adjustments as determined by survey results, patient participation records and supervisor’s direction. Prepares reports as directed by supervisor.
- Provides information and instruction on the proper use of exercise equipment to all participating patients, as appropriate. Maintains fitness equipment in association with the Policies and Procedures of the Recreation Department.
- Coordinates and facilitates with the activities of an-going federal diabetes grant. Responsibilities include, but not limited to, day-to-day record keeping, organizing of grant-related community events, collecting and tabulating of participants attendance, submitting collected data on a quarterly, half-yearly, or annual basis, and generating audit and required key measure reports per grant’s compliance guidelines:
- Participates in community-wide special events and education programs designed to prevent and control diabetes. Prepares materials for seminars, special events and for on-site reference needs for program participants and health care staff. Develops and presents information to organizations, using appropriate oral and written presentation skills.
- Attends meetings, workshops and training sessions to upgrade skills and maintain certification, as appropriate and as director by supervisor.
· Knowledge of the physiological implications of physical exercise and physical fitness.
· Knowledge of methods and techniques of aerobic exercise, water aerobics, walking and running for physical fitness.
· Knowledge of the basic techniques of CPR, First Aid, universal precautions for bloodborne pathogens and emergency referral procedures.
· Knowledge of medical terminology and policies for safeguarding confidential information.
· Knowledge of the symptoms and health risks of Diabetes Mellitus.
· Knowledge of Native American culture, history and traditions.
· Skill in establishing and maintaining patient records.
· Skill in the use of a personal computer and related software.
· Skill in establishing and maintaining effective working relationships with patients, supervisors, co-workers and representatives of resource agencies.
· Skill in writing for publication, including pamphlets, signs, bulletins and other educational materials.
· Skill in assessing and evaluating emergency situations and utilizing appropriate emergency procedures.
· Skill in evaluating patient needs and utilizing Wassaja Memorial Health Center and Ft. McDowell Yavapai Nation resources, or other appropriate referrals, to meet such needs.
· Ability to work non-standard hours.
· Ability to maintain accurate and timely records.
· Ability to successfully complete CPR and First Aid training and to maintain certification in these areas.
MINIMUM QUALIFICATIONS/EDUCATION:
- Bachelor’s degree in Physical Fitness, Physical Education, Exercise Science or closely related program and experience equivalent to one (1) year of full time work in a physical fitness program, preferably with experience in Diabetes prevention.
- Certification as a Physical Fitness Specialist or Certified Athletic Trainer (ATC) or Certified Strength and Conditioning Specialist (CSCS).
- Must possess certification in CPR or be able to obtain such certification within the probationary period.
- Current, unrestricted Arizona Drivers License.
