Job Type Full-timeDescriptionHarbor's Edge is a premier 5-star, resort-style Continuing Care Retirement Community (CCRC) located alongside the beautiful harbor in Norfolk, Virginia. Our community consists of 300 Independent Living Residential Units, a 33-bed (Medicare / private) Skilled Nursing Healthcare Unit, a 40-bed Assisted Living Unit, and 34-bed Memory Support Unit. We are seeking a highly qualified Licensed Master Social Worker (LMSW) who is responsible to help provide social services throughout the Community at Harbor's Edge, with a strong focus on healthcare. The LMSW position will serve as a resident advocate and ensure a full review of all pertinent criteria resulting in appropriate, timely assessments and accommodations for residents to facilities and services.The LMSW will effectively interact with residents and family members and is a vital member of the Interdisciplinary Team. Along with physicians, nurses, therapists, and dieticians, the LMSW will collaborate to comprehensively assess and address the needs of residents, help develop coordinated plans of care, and provide a holistic approach to meeting individuals' needs and goals. ESSENTIAL DUTIES AND RESPONSIBILITIES : Regularly assess the social, emotional, and psychological needs of residents on an ongoing basis. Ensure daily interactions with residents and family members are appropriate and all documentation is completed accurately and timely following all federal, state, local regulatory requirements, as well as policies and procedures of Harbor's Edge. Assist with admissions, transitions, development of treatment plans, and discharge planning processes. Provide resources and recommendations as needed. Meet with patients and their families for psychosocial needs assessment and examines social support, appropriate community resources, and coping mechanisms. Complete social service history and assessment for new admissions. In collaboration with the physician and nursing, provide education and assistance to patients and their families regarding Advance Directives and Resident Rights. Participate as active member of the Interdisciplinary Team (IDT) including Care Plan Meetings. Assess, evaluate, and document on social, psychological, and emotional needs of residents. Complete the Social Service portion of Minimum Data Set (MDS) to evaluate residents' health, update progress notes and resident care plans accordingly. Work with the healthcare team to promote positive adjustment and improve quality of life for our residents.Coordinate resident discharge planning processes and transitions. Obtain referrals for appropriate services including home care and Durable Medical Equipment (DME) prior to residents returning to the outside community.Establish and maintain productive relationships and positive communication with residents, family members, management, teams, staff, and community agencies, healthcare organizations, and other providers to promote a broad base of support for residents and family members. May assist with conducting resident satisfaction and discharge surveys.Perform case management services including resource coordination, develop, coordinate, and monitor a goal plan with residents and family members.Facilitate, and maintain support groups to enhance the quality of care provided to residents and their families.As directed by the physician, Direct or Nursing (DON) and / or Health Services Administrator (HSA), assist with conducting prompt, objective, thorough investigations and documentation relating to any resident or family complaint. Complete missing property investigations, documentation, and follow up, and assist with other facility investigations as needed.Provide grief and end of life counseling, obtaining referrals for hospice care as needed.Help ensure that staff understands residents' rights, and how to recognize and prevent abuse, neglect, and mistreatment.Model service excellence and serve as an effective resource for appropriate guidance, knowledge, and information. May work, when necessary, beyond normal working hours, on weekends, on holidays. and in other positions. Expected to report to work on time and work the full established schedule. RequirementsREQUIREMENTS : Master's degree from an accredited Council on Social Work Education (CSWE) program in Social Work. Fulfill supervised experience requirements for Licensed Master Social Worker licensure, and pass the Association of Social Work Board's (ASWB) exam.Current, unencumbered Licensed Master Social Worker in the state of Virginia.Minimum of two (2) years of social worker and / or case management experience in healthcare following completion of master's degree, including in depth knowledge of applicable healthcare regulations, preferably in a Continuing Care Retirement Community (CCRC) environment. Demonstrate knowledge of age-specific differences in the elderly resident population, including unique physiological, psychological, and developmental differences.Maintain current knowledge of organizational policies, National Association of Social Workers (NASW) Code of Ethics, and / or best practices within the healthcare setting.Excellent leadership, presentation, and interpersonal skills with effective verbal and written communication. Serve as role model for service excellence, and able to work effectively as a knowledgeable resource, member of the healthcare team, and independently.Demonstrate strong attention to detail, observation skills, organizational and time management skills, business acumen, high self-initiative, and ability to accurately follow and deliver verbal and written directives in a timely manner. Skilled at working with the elderly population in a courteous and friendly manner, demonstrating patience and compassion. Perform duties with consideration for residents' rights at all times, demonstrating integrity and confidentiality in the care of residents.Ability and competence to operate computer, applicable software programs (e.g., related medical documentation programs, electronic health record (EHR), scheduling, Microsoft Word, Excel, etc.), and other standard office equipment. Must be able to sit, stand for extended periods of time, stationary positions at certain times during the workday, walk, move about the facilities inside and outside, ascend / descend stairs, reach, turn, bend, stoop, crouch, kneel, balance, maintain body equilibrium to prevent falls, reach overhead, grasp with both hands, fine manipulation, coordination, and repetitive motion. Able to frequently exert up to 50 lbs. (unassisted) and / or lift up to 25 lbs. (unassisted) to move objects.
