Qualifications
- A bachelor’s degree and two years of clinical experience in the care of elders or people with disabilities
- A social worker license from the Massachusetts Board of Registration
The responsibilities of the care manager include:
- Conducting initial and ongoing psychosocial evaluation of a member’s appropriateness for AFC
- Selecting, training, evaluating, and supervising AFC caregivers in conjunction with the registered nurse
- Participating in the development, implementation, and ongoing review of the AFC plan of care
- Conducting on-site visits with each member at the qualified setting
- Reviewing AFC caregiver logs at a minimum of every 30 days
- Reporting changes in the member’s condition to the member’s AFC registered nurse
- Participating in admission, discharge and transition planning and implementation
