What are the responsibilities and job description for the LPN - PART TIME position at Fortcom Health Services?
Fortcom is currently looking to hire Part-Time LPN for Boston, Dorchester, Roxbury, Brockton, Lynn, Lowell, New Bedford, Worcester, Brockton areas of Massachusetts to perform home visits to clients of Critical Skilled Nursing program.
Job Responsibilities:
- Complete and coordinate all applicable clinical assessments and clinical evaluations
- Develop and review on an ongoing basis each member’s AFC plan of care
- Training, evaluating, and supervising AFC caregivers in conjunction with Program Director
- Reviewing the physician summary
- Monitoring the health status of all clients to ensure that all needed care is properly delivered
- Reporting changes in the health status of any member to the client’s physician
- Provide individualized on-site nursing care to clients in the comfort of their home
- Use a comprehensive and client individualized Plan of Care with innovative approach.

Education/Experience:
- LPN license (pass NCLEX-PN exam)
- Must be fully licensed RN by Mass Board of Registration
- At least two years of previous nursing experience
