  • Fortcom Health Services is currently looking to hire a Full-Time Program Director to manage and direct our Adult Family / Foster Care program.

    Reports To : Chief Operating Officer

    Licenses and Qualifications :

    The Program Director must have minimum of a BS in Nursing with RN License

    A Masters degree strongly preferred

    8 years of relevant experience preferably in a professional services environment

    Must have a drivers license and willing to drive to Members / Caregivers residency

    DUTIES : Direct the coordination of AFC patient care under the supervision of the COO.

    The responsibilities of the Program Director include :

    The Director of Adult Foster Care (AFC) is responsible for the successful administration and management of the clinical, budgetary and business operations of the AFC program. This role will provide leadership, supervision, guidance, and support to all program staff and is responsible for communication of all policies and procedures and require on-call availability. The Director for Adult Foster Care actively promotes community inclusion, self-advocacy, and individual rights. Some specifics responsibilities are below :

    • Development and implementation of the providers policies and procedures
    • Direct and supervise all aspects of the Adult Foster Care Program
    • Oversight of the hiring, training, supervision, firing, and evaluation, of all employees and independent contractors
    • Coordinate the payment of a stipend to all caregivers for their services
    • Participate in the fiscal administration of the program including billing, budget preparation, and required program statistical and financial reports
    • Ensuring that the AFC provider meets all of the requirements of MassHealth AFC Regulations
    • Work with COO to formulation of a business plan, i.e., strategic goals, resource allocation plan(s) and a yearly budget.
    • Evaluates the program, services, and field personnel.
    • Directs the implementation of service goals and objectives.
    • Staff development including orientation, in-service education and continuing education.
    • Oversees the implementation and evaluation of Individual care services inclusive of regulatory licensure and certification criteria and accreditation standards.
    • Coordinates with other program areas and senior management as appropriate according to the organizational structure.
    • Collaborates in the development of an effective and efficient Individual care documentation system including statistical compilation and analysis relative to cost and staff service delivery.
    • Problem solving at the organization and community levels.
    • Oversees all CQI and utilization review activities.
    • Assists in marketing the Agency and fostering community relations.
    • Participates in state and national organizations, meetings, seminars, workshops and activities relating to the health professions and health care services, when appropriate.
    • Assists with interviews and hires select Caregiver personnel.
    • Assist in Accreditation compliance and readiness.
    • Performance of other duties that may be assigned by the COO and Management.
