Social Worker Consultant Job Description
Forward Action Recruiting and Staffing is seeking a Licensed Medical Social Worker Consultant for a position located in Fort Worth, TX. The ideal candidate will have experience in Long-Term Care (LTC) and Patient Care Coordination (PCC) and be able to start immediately. This position requires travel.
Responsibilities:
- Provide social work consultation services to clients in LTC facilities
- Assess and evaluate the psychosocial needs of clients and develop care plans accordingly
- Collaborate with interdisciplinary teams to ensure the best possible care for clients
- Provide education and support to clients and their families
- Document all services provided in a timely and accurate manner
Requirements:
- Licensed Medical Social Worker (LMSW)
- Experience in Long-Term Care (LTC) and Patient Care Coordination (PCC)
- Ability to start immediately
- Willingness to travel
