Job Title: Social Care Network (SCN) Partner Liaison Specialist

Reports To: SCN Project Director

Project Summary:

The New York State Department of Health has established Social Care Networks (SCNs) as part of the 1115 Waiver Demonstration Amendment, and Forward Leading IPA (FLIPA) is the SCN Lead for the Finger Lakes Region. This region is comprised of 14 counties, including Allegany, Cayuga, Chemung, Genesee, Livingston, Monroe, Ontario, Orleans, Schuyler, Seneca, Steuben, Wayne, Wyoming, and Yates. The SCN’s aim is to enhance the delivery of social care services to Medicaid members by coordinating efforts among community-based organizations (CBOs) and other health care partners to create a more resilient, flexible, and accessible social care system that reduces health disparities and advances health equity. The SCN will collaborate with CBOs and other health care partners, leveraging shared data and technology to coordinate social care services for Medicaid members to improve access, ensure reliable and timely referrals, streamline and track navigation and completed referrals in closed loop systems and enhance collaboration between social care service providers and other regional partners.

Job Summary

The SCN Partner Liaison Specialist (PLS) will support approximately 8-10 CBOs at any time in onboarding and successfully implementing required SCN activities. Each PLS will be responsible for supporting CBOs with all aspects of assessment, plan development, securing required technical assistance, reporting, and other operational implementation support. This position is grant-funded through March 2027.

FLIPA is renowned for its commitment to integrating primary care, behavioral health, and social care needs. Our growing membership serves individuals across more than 27 counties and includes federally qualified health centers (FQHCs), behavioral health providers, and a rural health network consisting of eight county public health departments.

Duties/ Responsibilities:

  • Build relationships with assigned CBOs to navigate the SCN readiness process
  • Utilize the standardized approach to CBO onboarding and implementation while tailoring as needed to meet CBO-specific needs
  • Support CBOs to complete a readiness assessment and create a customized Dynamic Needs Map to outline actions required prior to systems onboarding
  • Provide project management support to CBOs to facilitate completion of Dynamic Needs Map actions including but not limited to development of project plans, creation of workflows, and development of resources and tools
  • Provide technical assistance to CBOs to build capacity and readiness to participate in screening and navigation and/or service delivery for the SCN
  • Ensure CBOs access or participate in relevant technical assistance activities
  • Serve as an organizational change management support for CBOs
  • Provide ongoing support and monitoring of onboarded CBOs leveraging reports/dashboards to ensure progress towards required milestones
  • Provide regular transparent communication and reporting of CBO progress and needs to the SCN Project Director and other FLIPA SCN leadership
  • Actively participate in shared learning with other Partner Liaison Specialists and FLIPA SCN team members via group-based peer learning to accelerate improvement and ensure effective communication and continual forward progress
  • Support FLIPA SCN leadership to understand CBO needs to drive development of additional or tailored technical assistance supports as required
  • Support implementation of learning community strategy to facilitate knowledge sharing, development of standardized approaches and continuous improvement strategies across SCN network CBOs and service providers
  • Support data collection and project reporting activities and effective communication of dashboard/reporting findings
  • Identify challenges and opportunities for improvement related to project implementation and CBO relationship management
  • Effectively work across multiple organizations and geographies to support project progress within different organizational culture structures and communities
  • Other duties as assigned

Education & Experience:

  • Bachelor’s/Master’s degree in related field, such as business, public health, healthcare administration or equivalent experience required
  • 3 years of relationship management, project management or related experience required
  • Prior experience with large-scale implementation grants/projects preferred
  • Experience within the New York State health care system (population health, behavioral health, Federally Qualified Health Centers, or similar) required 

Skills, Knowledge, and Abilities:

  • Excellent organizational, leadership and project management abilities
  • Ability to manage multiple priorities effectively with minimal supervision 
  • Familiarity with regional CBOs, health care systems, and partners preferred
  • Desire to make a lasting impact in underserved communities with a strong commitment to equity and social justice
  • Outstanding communication and people skills 
  • Demonstrated ability to build relationships and support organizations to drive lasting change
  • Ability to gather information throughout the lifespan of a project and report out to a variety of audiences
  • Experience seeing project through the full life cycle
  • Strong interpersonal skills, independently resourceful and thrives in a fully remote environment
  • Proven ability to complete projects according to defined timelines and objectives
  • Proficient with MS Office and various business software 

Salary Range:

  • Salary is commensurate to education and experience with range of $55,000 – 75,000.

Additional information:

  • This position will be primarily remote. However, given the nature of the role, approximately 50% of the workday is anticipated to include regular travel and in-community work within the Finger Lakes region for community convenings and relationship building. Occasional travel across New York State is also expected for on-site meetings or training.
  • Residency within the Finger Lakes region of New York State is required.

Pay: $55,000.00 - $75,000.00 per year
