A Field Nurse Case Manager here at ForzaCare will work collaboratively and closely with the injured worker, insurance carrier, employer, physician, and various parties, to provide a coordinated plan of care, facilitate the injured workers recovery, with the ultimate goal of a successful return to work.
You will: (ESSENTIAL FUNCTIONS)
- Communicate with medical providers, customers, employers, attorneys and injured workers both in person and over the phone.
- Document actions, plans and goals in emails and in reports to customers using different forms and letters in the case management process.
- Collaboratively develop recovery goals and a plan of care, ensuring injured workers’ compliance, orchestrating medical care and return to work efforts.
- Develop lasting relationships with customers, medical providers and ForzaCare team members.
- Ensure the case management work provided meets or exceeds customer requirements and positively impacts the cases you are assigned.
- Attend appointments with the injured workers, which may include visiting employers and injured workers at their place of employment.
You are: (ESSENTIAL QUALIFICATIONS)
Qualified and Experienced (required experience, education, certifications, licensure, and/or registrations):
- A RN licensure and/or a Certified Rehabilitation Counselor with associated college degree is required.
- Must comply with licensure and/or certification (CCM, CIRS or other Case Management certifications) requirements within the state(s) in which you practice is required.
- Must maintain all licensure and/or certification requirements within the state(s) in which you practice is required.
- Experience as a worker’s compensation case manager is preferred.
- You have the ability to travel using a valid driver’s license, reliable transportation and auto insurance.
