CASE COORDINATOR 

Job Status: Full-time

Reports to: BI Program Manager

Responsibilities:

Case Management

  • Family satisfaction re: proper support, advocacy, resources, communication
  • Complete assessments: scoring, tracking progress, to serve clients' needs
  • Attend court appointments & other meetings
  • Work with BI team in matching BIs to homes
  • Review Incident Reports
  • Facilitate Referral Triage, Intake, Meet & Greets
  • Complete/submit Recommendation & Approval Letters for services
  • Create Individualized Behavioral Plan for each client; review & update
  • Provide updates to BI Team & families re: intake process
  • Complete quarterly meetings with families
  • Write/submit renewals
  • Update client database of changes
  • Submit End of Month Reports
  • Complete Case Closure & Aftercare Follow-up meetings & documentation
  • Review BI Quarterly Summaries & provide feedback to staff

Administrative Leadership & Supervision  

  • Employee satisfaction: build a strong sense of team, providing support & advocacy
    • Work with BI Team in the on-boarding process
    • Assist in training all new BIs & All-Staff Meetings
    • Conduct client/staff matchings with Program Manager
    • Provide regular/on-going evaluation of BIs
      • Provide in-home supervision for BI staff
      • Track all missed Shift Notes until completed
      • With Program Manager, complete corrective actions & terminations
      • Update/track employee timecards
      • Provide support to BIs during OHI Investigations
      • Work with colleagues of all demographics to serve clients
      • Build relationships with partner agencies & community stakeholders
      • Maintain strong, open communication with BI Team
      • Maintain CPR/First Aid certification
      • Work collaboratively with branch office(s) to ensure program fidelity
      • Attend all required professional development trainings

      Outcome Planning/PQI  

      • Complete agency reports & statistics
      • Establish procedures to track/report program outcomes & quality assurance
      • Maintain compliance of resource provider files
      • With the Program Manager, complete Critical Incident Reports

        Qualifications & Preferred Skills  

        • MA/MS, preferred; (or BA/BS 2 years in non-profit social service) in social work, psychology, public admin, or related human services field, required
        • Experience working with at-risk populations
        • Familiarity with the child welfare system
        • Resourcefulness in setting priorities, creating efficiencies, guiding investment in people & systems
        • Understand program development, implementation & evaluation
        • Organizational & planning skills
        • Commitment to principles of support & advocacy for foster & adoptive families
        • Strong verbal & written communication skills
        • Leadership ability, team management, interpersonal skills; establish rapport & build trust
        • Advocate for positive outcomes within multidisciplinary &  diverse professional teams
        • Flexible, self-starter, multi-tasker, detail-oriented, can meet deadlines
        • Team player
        • Model ethical & professional behavior
        • Participate in required ongoing meetings/professional memberships & trainings
        • Experience working with marginalized communities with a nuanced understanding of systemic oppressions
        • Flexibility, adaptability working with a diverse group

          Salary : $50,000 - $52,000
