Job description
JOB DESCRIPTION:
The person filling this position will be responsible for working with adults who have been impacted by the criminal justice system, struggling with substance abuse and mental health and reentering their communities after incarceration. The Case Manager will assist and complete follow up phone calls to the participant to schedule an intake/assessment within forty-eight hours of receiving a referral. The Case Manager will work to engage with and assist program participants with challenges and barriers they may encounter while navigating through the continuum of reentry supports and services.
1. PRIMARY JOB DUTIES
· Oversee a caseload of adult participants
· Provide intake, assessments, orientation to the program and referrals as needed
· Work with participant to assess the appropriate mode of service for participants
· Utilize trauma-informed, housing-first, motivational interviewing, and harm reduction approaches in service delivery.
· Check in with participants weekly to monitor participant well-being and ongoing needs
· Collaborate with law enforcement, probation officers, the district attorney’s office, and community service providers
· Identify emergency resources, harm reduction options, housing and employment, etc.
· Assist clients with substance use, medical, and mental health, housing, employment
· Maintain detailed client records, including demographics and contact notes
· Attend required weekly and monthly meetings and participate in relevant community initiatives and collaborative efforts
· Prepare and submit accurate monthly reports
· Complete 3 month and 6 month review of cases to determine progress and necessity of ongoing program participation
· Demonstrate a clear knowledge of the agency’s established policies and procedures, objectives and quality assurance as measured by case and file reviews with the supervisor
· Complete all required training within 3 months of hire.

· maintain flexibility and creativity in working a non-traditional schedule
3. Professional development
To assure continuous professional growth and development, the Reentry Case Manager will:
· actively participate in individual and/or group supervision and staff meetings
· attend and participate in all required and other professional trainings
· independently seek out resources and learning opportunities to foster ongoing professional growth
Essential Qualifications
· Bachelor’s degree in human or social services or related field preferred
· At least two years of supervised experience in case management or a related field working with high risk youth and or young adult populations. Must have knowledge of the nonprofit and human service landscape and referral networks, and must be culturally competent.
· Must be able to obtain all applicable clearances
· Valid State Driver’s License and insurance
