Qualifications :
1. Bachelor's Degree from an accredited college or university
- Master's Degree preferred
2. A valid Colorado Teaching Certificate endorsed or Highly Qualified in area of assignment
3. Two years of experience working with individuals with special needs and / or agencies supporting individuals with special needs preferred
4. Such alternatives to the above as the Board may find appropriate
Job Goals :
Responsible for planning, implementing, and evaluating instructional activities and work / volunteer opportunities for students in Fountain-Fort Carson School District 8's Post High School Transition Program. Create a flexible class environment favorable to learning and personal growth; establish effective rapport with students; motivate students to develop skills, attitudes, and knowledge in accordance with each student's post-secondary outcome goals; and establish good relationships with parents and other staff members. Establish effective rapport with Transition Manager and Job Coaches, as well as instructional teams within Fountain-Fort Carson High School.
Performance Responsibilities :
1. Work collaboratively with Transition Manager to coordinate community outings, workshops by community members, and work / volunteer experiences
2. Interface regularly with SWAP and DVR to maximize supports and opportunities for students
3. Identify student needs, make appropriate referrals, and develop goals for individualized education plans (IEPs).
4. Develop lesson plans, implement lessons, and evaluate instructional activities that support individual goals.
5. Evaluate students' growth, keep appropriate records, and prepare progress reports.
6. Plan, coordinate, and oversee the day to day work of job coaches in the classroom and in the community.
7. Communicate effectively with parents by means of phone calls, conferences, etc.
8. Work collaboratively with high school special education team to determine appropriateness of Transition services for students and facilitate transition
9. Work directly with high school administrators and special education team to vertically align vocational, recreational and life skills / concepts taught to students
10. Foster and enhance relationships with community partners to provide a vast array of work / volunteer / experiential opportunities for students with special needs aged 18-21
11. Strive to maintain and improve professional competence by attending inservices, conferences, and trainings
12. Drive students in district provided vehicle to community outings
13. Provide medical and personal care to student as needed and in accordance with training provided by nursing and / or other staff as needed.
14. Perform other duties as assigned
Salary :
Certified salary is based upon degree status and years of experience. Horizontal advancements are based upon "Graduate" level credits and staff development hours obtained after the last degree was conferred.
Salary Schedule
One year of credit will be provided for each year of approved outside teaching experience.
