Patient Navigator – Primary Care Integration Specialist – This position is responsible for establishing and maintaining referral relationships with primary care and other healthcare providers. This includes establishment of referral and follow-up protocol and exchange of meaningful healthcare information, management of grant project data and completion of all required activities to maintain grant funding based on CCBHC criteria (Certified Community Behavioral Health Clinic).
This requires both interpersonal and data management skills. The patient navigator assures that patients are connected with the right programs and services and seeks to close referral gaps that patients may encounter while receiving care in the CCBHC clinic. The patient navigator must be able to work collaboratively with clients, families, internal programs, external providers, and all aspects of the CCBHC network. The patient navigator must be an outcome focused individual that seeks to help clients have an effective and efficient experience while receiving care. This position is geared towards increasing access to and improving the quality of community mental health and substance use disorder services in pursuit of healthier communities.
A Bachelor’s degree in healthcare or human services field, Registered Nurse, or a minimum of 4 years practice as a case manager or care coordinator in healthcare setting is required.
Hourly position: starting wage is $21.15/hour.
