What are the responsibilities and job description for the Patient Access Coordinator position at Four Rivers Community Health Center?
General Summary : A non-exempt position, Medical Assistant or LPN, providing indirect patient care in the office / clinic setting. The Patient Access Coordinator is the first voice of Four Rivers Community Health Center that patients, physicians and community members will encounter when communicating with the organization.
Essential Duties :
- Work cooperatively with Director of Medical Services and personnel to accomplish all goals and objectives of Four Rivers Community Health Center.
- Manage large amounts of inbound clinical calls in a timely manner .
- Identify patient needs, answer questions, and solve problems.
- Retrieve recorded messages and distribute to appropriated individuals; place telephone encounters and assign as appropriate .
- Document in real-time all patient phone encounters.
- Responsible for insuring patients receive their prescription refill requests , via internal & extern al fax, telephone encounters, WELL App, etc , within 48 hours or requesting .
- Responsible for assisting patients with referrals to specialists, laboratory testing, etc.
- Keep work area neat and organized for proper completion of work tasks.
- Participate in Continuing Education as required by supervisor or state or federal guidelines; and,
- Perform related work as required, including, but not limited to, assisting, and fulfilling duties as needed.
