Required qualifications
A high school diploma or GED equivalent and the successful completion of the 20-hour social service course or other social worker designee program within the state. Note: a person who has not completed a social service course shall receive consultation from a person who meets the qualifications until he/she has satisfactorily completed the approved course.

Duties And Responsibilities
- Implement the admission process for new residents.
- Develops an initial care plan based on the physical and psychological needs of the resident.
- Continuously updates resident care plans and documentation regarding resident condition.
- Provides direct counseling to residents, family members, and/or resident representatives.
- Works closely with the healthcare team to identify, assess, and address any needs that can be met either through social services, a chaplain, or a mental health provider.
- Assists the resident in adjusting to the community and promotes continuity of relationships with family and community.
- Coordinates relocation planning, including advising and referring to community resources before and during relocation. Coordinates transfers of residents from the residential building to the health center.
- Establishes, advises, and implements policies to ensure the safeguarding of confidential information obtained in the compilation of residents' social histories and assumes privacy of interviews.
- Participates in the quality assurance process as assigned by the director of nursing.
- Knows the RAI process and performs MDS assessments as needed.
- Educates staff regarding the psychosocial and behavioral needs of residents.
- Performs other duties as assigned.
