Position Overview
Fourth Street Clinic seeks a therapist (CSW, ACMHC, LCSW, or CMHC) interested in working in a progressive behavioral health program. An integrated medical and behavioral healthcare team ensures patients access to various services. Behavioral health services focus on screening, assessments, individual and group counseling, and medication management. Therapists act as treatment providers and advocates, helping clients gain access to resources while addressing their behavioral health needs, including mental illness and substance use disorders. Clinical supervision will be provided.
Duties & Responsibilities

- Serve as an integrated team member and work to ensure patients have access to whole-person care, including medical, pharmaceutical, and dental services.
- Provide individual and group therapy for patients with mental health and substance use disorders.
- Perform ASAM and biopsychosocial assessments to inform treatment planning and referrals to outside entities for higher levels of care when appropriate.
- Assist clinic staff and work with clients who present in crisis by providing crisis management, assessing safety, and creating safety plans.
- Assist the primary care team in developing care management processes such as using guidelines, disease management techniques, case management, and patient education to improve chronic disease self-management.
- Provide assessment, consultation, and brief intervention for mental health concerns.
- formulate appropriate mental health diagnoses.
- Assures quality of care by adhering to evidence-based practice, therapeutic standards, and standards of care set by DOPL.
- Maintain accurate and timely documentation of all clinical encounters in the clinic's electronic health record.
- Follow HIPAA and 42 CFR confidentiality guidelines.
- Maintain professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal networks, and participating in professional associations.
Necessary skills/attributes
- Competency in multiple psychotherapy modalities and evidence-based theoretical frameworks including but not limited to harm reduction, trauma-informed care (TIC), cognitive behavior therapy (CBT), seeking safety, acceptance and commitment therapy (ACT), motivational interviewing, and medication-assisted treatment (MAT).

Supervisory Relationships
This position reports to the Behavioral Health Director
Qualifications
- Graduate of an accredited master's program in Social Work or Mental Health Counseling, or equivalent degree, and licensed in the State of Utah
- Bilingual English/Spanish desirable
Salary : $66,000 - $70,000
