The Medical Case Manager plays a pivotal role within the Medically Vulnerable People's (MVP) team, serving as a compassionate advocate for patients in need. Working onsite at our MVP program in Sandy, for the unhoused and medically vulnerable individuals in our community, they collaborate closely with the healthcare team to identify vulnerable patients and conduct thorough needs assessments. With a patient-centered approach, they create and maintain care plans tailored to individual needs, coordinating medical, behavioral health, and support services both within the clinic and with community partners. Their dedication to continuity of care, accessibility to services, and addressing social determinants of health ensures that patients receive comprehensive support as they navigate their healthcare journey.
