The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers.  To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives implemented by DOHMH to advance community health throughout the city. It facilitates partnerships, often new and unconventional, between government and the private sector to develop, test, and launch new initiatives. These collaborations speed the execution of demonstration projects, affect expansion of successful pilot programs, and support rapid implementation to meet the public health needs of individuals, families, and communities across New York City.   PROGRAM OVERVIEW   The Bureau of Harlem Neighborhood Health (Harlem BNH) supports the agency’s mission to protect and promote the health of all New Yorkers.  Harlem BNH catchment area includes the entire community of Harlem, including East, Central and West Harlem as well as Washington Heights and Inwood.  Harlem BNH focuses on the structural and root causes of health disparity outcomes and develop programs to address these outcomes with input from partners and residents.  Addressing the social determinants of health (SDOH), is a key focus point and is enshrined in programs such as the Harlem Health Advocacy Partnership and the Asthma Counselor Program.  Other Bureau offerings include a variety of programs and activities focused on the health and wellness of Harlem residents. The Harlem Bureau also houses the East and Central Harlem Neighborhood Health Action Center, which is a key part of the NYC's effort to promote health equity and reduce health disparities in the neighborhood.   As part of the NYC DOHMH Public Health Corps project, the Harlem BNH team has Community Health Workers supporting all of its programs and initiatives.   POSITION OVERVIEW   Community Health Workers at the Bureau of Harlem Neighborhood Health (Harlem BNH) are frontline public health workers who provide outreach, education, advocacy and informal counseling to community members with the goal of bringing essential services to the community.   Harlem BNH seeks to hire a Health Equity & Capacity Building CHW to assist in delivering capacity- building support to the community and Bureau partners, including, but not limited to, recruitment and engagement of participants, engagement of partners, gathering program data, and informing the development and evaluation of programs.   RESPONSIBILITIES   Heath Equity Capacity Building Work with HECB Sr. Managers to develop and operationalize programs to address health disparities and inequities that align with community and bureau goals and priorities, including: Conducting outreach. Counseling participants on programs and resources. Providing referrals to community services. Working with the Bureau R&E team in data gathering, reporting, and assessment. Developing professional relationships with community organizations, businesses, DOMHN, and other key stakeholders. Support the HECB and Bureau program teams in trainings and workshops for community-based organizations. Support Sr. Managers in identifying new resources and interventions for implementation in East and Central Harlem. Facilitate and deliver presentations at community meetings and events. Perform other related duties as assigned.   General Support community members in advocating for their needs. Meet with community members in groups and one-to-one settings to support assigned program responsibilities. Deliver health information in a way that is easy to understand. Determine eligibility for public programs and services, as needed. Collect data to inform future programs. Facilitate communication between community members and Harlem BNH and DOHMH Respond to emerging community priorities as part of the CHW workforce, as needed.   QUALIFICATIONS   Community engagement experience (e.g. outreach, education, etc.) Data collection and data entry experience Problem-solving skills Spanish language fluency required Experience embedding racial equity and social justice lens in data work Very strong organizational skills and ability to handle multiple priorities  Excellent written and oral communication skills  Self-motivated and self-disciplined, with a strong sense of teamwork and the ability to function effectively both independently and as part of a diverse team of public health professionals  Working knowledge of Microsoft applications, including Word, Excel, PowerPoint, and Outlook Ability to work evenings and weekends, as needed.   SALARY AND BENEFITS   FPHNYC offers a comprehensive benefitpackage  Salary range is $55,000to $60,000  Public Service Loan Forgiveness (PSLF) eligible employer  Generous Paid Time Off (PTO) policy  Medical, dental, and life insurance with low or no employee contribution  A retirement savings plan with generous employer contribution  Flexible spending medical and commuter benefits plan  Meaningful work at an organization striving to advance health equity and social justice   ADDITIONAL INFORMATION    9:00am – 5:00pm Monday to Friday Hybrid   Location    NYC DOHMH Public Health Corps, 161-169 E. 110th Street, NY, NY 10029      All new hires must be vaccinated against the COVID-19 virus unlessthey have been granted a reasonable accommodation for religion or disability.
