The Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers.  To this end, in partnership with the New York City Department of Health and Mental Hygiene (NYC Health Department), FPHNYC incubates innovative public health initiatives implemented by NYC Health Department to advance community health throughout the city. It facilitates partnerships, often new and unconventional, between government and the private sector to develop, test, and launch new initiatives. These collaborations speed the execution of demonstration projects, effect expansion of successful pilot programs, and support rapid implementation to meet the public health needs of individuals, families, and communities across New York City.   POSITION OVERVIEW The NYC Health Department is seeking a Community Health Worker to provide point-of-care and family education for pediatric asthma self-management and support. The Center for Health Equity & Community Wellness (CHECW) seeks to eliminate racial and other inequities resulting in premature mortality. With an unwavering grounding in history and structural analysis, CHECW works to increase visibility of the harm perpetuated by centuries of racist, socially unjust policy while pushing towards redress for the most impacted NYC communities. CHECW addresses inequity across community and healthcare systems in partnership with community, faith-based, and health care organizations. CHECW's work focuses on social determinants of health, including environmental and commercial determinants, and addresses both upstream and downstream factors to improve health and well-being of New Yorkers.   PROGRAM OVERVIEW The Bureau of Bronx Neighborhood Health, which is under CHECW, aims to enhance health, wellness and equity in the Bronx to elevate quality of life and life expectancy by: Creating targeted programs to address community health needs and concerns.​  Supporting and empowering community members and organizations​.  Generating and sharing public health research and data to champion the borough's health.​  Advocating for policy reforms that will positively impact the health of the Bronx​.   This position is grant-funded.    RESPONSIBILITIES Assist with planning and executing health awareness and promotion activities in diverse settings (some evening and weekend work required). Engage and provide health education and information to community partners and residents. Connect individuals to appropriate referrals and NYC Health Department services. Conduct outreach to community residents, organizations, businesses, and other partners to help control asthma and other health conditions. Present to key stakeholders, community partners and residents about asthma support resources offered by the NYC Health Department via presentations and one-on-one meetings. Serve as a liaison between the NYC Health Department and other providers to optimize coordination of care. Assess for health-related social needs and refer as needed to programs such as: Integrated Pest Management (IPM) Refer to internal or external case management services within the SBHC’s health system or NYC Health Department-funded organizations when appropriate. Document each pediatric encounter in detail in the electronic medical record. Collect and summarize data to monitor patient touches, changes in outcomes. Take part in public health detailing initiatives in the South Bronx when needed. Performs other duties as assigned.   QUALIFICATIONS High school diploma or GED Associate degree or bachelor’s degree in community health or related field preferred. Bilingual (English/Spanish or English/French) is required. Strong relationship builder with excellent interpersonal and communication skills.  Experience providing coaching, mentoring, adult learning, counseling, outreach, workshop facilitation, and/or group/community presentations preferred.  Experience working with marginalized populations preferred.
