WHAT YOU CAN EXPECT
Investigates and determines patient medical insurance coverage.
Provides patient reports as necessary.
Communicates with referral sources including hospital, ACO, physician practices, payers, case managers to gain the necessary information for providing patient care.
Identifies and recommends the utilization of other community resources when appropriate.
Manages information related to home health and hospice programs, services and reimbursements; provides support or clarification to patients or staff as issues arise.
Assists and educates patients, families, physicians, and clinicians with questions, clinical issues or other problems to facilitate the transition to home or hospice care.
Coordinates initial physicians' orders with staff in central intake.
Collaborates with patients, families and other healthcare personnel or outside agencies regarding coordination of care and discharge planning.
QUALIFICATIONS
Associate's Degree- Nursing- Required
Bachelor's Degree- Nursing- Preferred
2 Years Home Health or Hospice Care- Preferred
2 Years Acute Care or Long Term Care- Preferred
Registered Nurse (RN) State Licensing Board- Required
Cardiopulmonary Resuscitation (CPR)- American Heart Association- Required
Auto Insurance- Required
Driver's License- Required
