Job Summary
We are seeking a compassionate Licensed Practical Nurse (LPN) to join our team. As an LPN, you will provide high-quality patient care based on physician orders and established nursing plans.
Key Responsibilities
- Implements nursing plan of care at the direction of the registered nurse.
- Provides home-based patient care based on physician orders and the established nursing plan of care.
- Reports any change in patient status to the Home Health RN and documents as appropriate.
Collaboration and Teamwork
You will collaborate with the Home Health RN and multidisciplinary treatment team to individualize each patient's treatment needs and achieve defined goals for improved patient outcomes.
Qualifications
- Associate's Degree or Diploma from Accredited Licensed Practical Nurse Program - Required
- 3 Years Acute Care or Long-Term Care Experience - Required
- 1 year Home Health Experience - Preferred

- Cardiopulmonary Resuscitation (CPR)- American Heart Association- Required
- Driver's License - Required

Travel Requirements
Up to 75% travel is required for this position.
