We are seeking a skilled and compassionate Nurse Care Coordinator to join our team at Franciscan Ministries. As a key member of our interdisciplinary team, you will play a vital role in ensuring the highest quality care for our residents.
The primary purpose of this position is to plan, organize, and direct the Minimum Data Set (MDS) process in accordance with current applicable federal, state, and facility standards, guidelines, and regulations. This includes conducting and coordinating the development and accurate completion of the resident assessment (MDS, CAAS, and Care Plans), as well as monitoring all related reports.
You will be responsible for communicating essential information to the Interdisciplinary Team/Management Team and participating in Quality Assurance and Accreditation (QAA) processes as needed. Your attention to detail and ability to verify accurate completion of resident face sheets, ranking diagnoses, and coding on admission and throughout the MDS process will be crucial in ensuring accurate reimbursement.
To be successful in this role, you must have an Associate's or Bachelor's degree in Nursing and possess 1-2 years of experience in MDS. You should also be able to demonstrate compassion when working with residents and maintain confidentiality at all times.
