What are the responsibilities and job description for the Clinic - Population Health Nurse position at Franklin County Memorial Hospital?
Summary : Promote effective partnerships between patients, nurses, physicians, other qualified healthcare providers and clinical disciplines to coordinate care of patients with chronic diseases and effectively manage care transitions. Will partner with the provider care team for successful preventative care visits to reduce the severity of chronic disease and avoidable acute illness and provide clinical health coaching to assist patients with self-management of their chronic disease.Minimum QualificationsEducation : Current License RN / LPN in the State of MS, BLS certifiedExperience : Previous experience in caring for chronic disease patients required. Two years of experience in provider practice or clinic health setting preferred. Previous Care Coordination, Case Management, Home Health or Behavior Health experience preferred.Knowledge / Skills : Possesses strong clinical assessment and critical thinking skills necessary to develop a comprehensive plan of care appropriate to patients with complex medical, emotional and social needs. Demonstrates evidence of effective organizational, leadership, communication, education, collaboration and counseling skills. Ability to work as a team-member and without direct supervision. Advanced computer skills. Demonstrates understanding in use of IT resources and patient databases. Professional practice behavior when working with public and fellow associates.Physical and Mental Effort : Minimal to strenuous physical exertion with some bending, stooping and lifting. Must have ability to concentrate on detailed jobs, handle interruptions and meet deadlines in a busy office situation. Moderate stress in dealing with busy office schedules and the public. Requires good communication skills, courtesy, discretion and confidentiality in all aspects of work.Environmental and Working Conditions : Normally works in a comfortable office setting. Constant contact with the public. Subject to exposure to infectious conditions and biohazardous materials. Typically 40 hours per week with overtime as needed.
