JOB SUMMARY:
The Referral Specialist participates in coordination, planning, implementation and evaluation of care provided by their assigned patient care teams by ensuring that referrals generated by the care teams are ordered, scheduled, completed or initiate appropriate patient follow up if appointment not kept. The referral specialist ensures that documentation is received by the care team from the specialty or referred provider. The referral specialist will track all ordered tests and referrals until the referral cycle is completed for their assigned care teams. The Referral specialist effectively communicates with assigned care team both verbally and by appropriate documentation in the patient’s record. The referral specialist maintains a pleasant, courteous and cooperative manner with patients, visitors and Center's personnel.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Participates as appropriate in Care team interventions. Communicates regularly with assigned care team members.
- Facilitate timely messages for the care team by accurately recording information in the EHR system and tasking information to the correct individual.
- Maintains confidentiality at all times.
- Review and assess patient records for referrals and diagnostic tests to ensure orders are initiated.
- Assists care team as necessary.
- Documents properly all actions taken to complete the tracking cycle.
- Effectively utilizes all reports and technology (EHR, Azara etc…) to ensure that tracking is efficiently carried out.
- Ensures coordination and tracking of all referrals and diagnostic tests as required.
- Involving the patients in activities to improve their health (patient engagement) Uses skills for patient assessment and implements patient teaching/education/ self-management.
- Participates in Care team quality improvement activities including care team performance on required clinical, operational and financial measures.
- Referral specialist will obtain appropriate prior authorization as necessary to assist care teams in making successful referrals.
- Referral specialist performance will be based on goals set for referral tracking completion.
- Participate in the appropriate training sessions, maintain CEU's as required by the State Nurses Licensure Board if applicable.
- Answers telephone in a courteous, professional, and efficient manner.
- Utilizes RTS system to verify patients insurance at each visit. Update the insurance information as changes are identified.
- Supports provider meaningful use efforts by collecting necessary information such as but not limited to pharmacy utilized by patient, email address, other providers seen, etc.
- Cross training in other call center areas and will be expected to work in other call center areas as necessary.
- EDUCATION AND EXPERIENCE:
High school graduate or GED equivalent. Graduate of an approved Medical Assistant Program or an equivalent combination of training, education and work experience relative to the position. Must have pleasant personality, good telephone etiquette, and public relations skills. One year or more of work experience in a medical setting with knowledge of medical terminology, CPT coding, and Medicaid, Medicare, and other third party insurance carriers billing experience.

- Medical Office Experience: 1 year (Preferred)
