Position Summary:

As an Admissions Nurse, you will play a critical role in ensuring a smooth and efficient admissions process for residents entering our skilled nursing community. This position blends clinical expertise, administrative competency, and business acumen to assess referrals, determine clinical appropriateness, optimize reimbursement, and ensure compliance with regulatory and organizational standards.

The Admissions Nurse is a key member of the clinical leadership team, participating in the Health Services on-call rotation and collaborating across departments to support census management, operational efficiency, and high-quality resident care.

This role is 70% clinical and 30% administrative, requiring a hands-on approach to resident assessment and placement while maintaining a deep understanding of reimbursement models, payer sources, and regulatory requirements.

Essential Responsibilities

Clinical & Admissions Responsibilities (70%)

  • Evaluate referrals for clinical appropriateness, ensuring Frasier can meet the prospective resident’s medical and personal care needs.
  • Conduct pre-admission assessments, including reviewing medical records, medication lists, physician orders, and social histories.
  • Identify and anticipate potential challenges, including high-acuity needs, infection control risks, and medication management requirements.
  • Assess acuity levels and collaborate with nursing leadership to ensure appropriate placement that aligns with clinical capacity and team resources.
  • Determine cost of care and its impact on reimbursement, collaborating with leadership on financial feasibility.
  • Ensure smooth transitions into care, coordinating with physicians, nurses, therapists, and families to develop a comprehensive plan of care.
  • Support the clinical team with resident care—particularly during periods of census stability—by assisting with assessments, interventions, and care coordination.

Regulatory & Financial Responsibilities (30%)

  • Navigate payer sources, including Medicare, Medicare Advantage Plans, Medicaid, and private insurance, ensuring appropriate authorizations and documentation.
  • Correctly apply ICD-10 coding and the Patient-Driven Payment Model (PDPM) to ensure accurate reimbursement and financial viability.
  • Ensure compliance with all regulatory requirements, including obtaining consents, verifying vaccinations, and completing care planning documentation.
  • Maintain accurate admission records and documentation in the Electronic Health Record (EHR), ensuring adherence to state and federal regulations, and communicating essential information for efficient and outstanding resident care.
  • Educate residents and families on financial and care expectations, fostering transparency and trust in the admission process.

Operational & Leadership Responsibilities

  • Work in partnership with the VP for Health and Supportive Services, finance team, social services, and clinical/rehab team to streamline the admissions process and optimize operational efficiency.
  • Monitor census trends and collaborate with leadership to develop strategies for maintaining budgeted occupancy levels.
  • Participation in the on-call rotation is an expectation as part of the nursing leadership team, providing support for admissions and urgent clinical needs as they arise.
  • After-hours and weekend work may occasionally be required to facilitate timely admissions, ensuring smooth resident transitions and optimal census management.
  • Contribute to quality assurance and performance improvement (QAPI) initiatives, identifying opportunities to enhance the admissions process, and the provision of care in our setting.

Qualifications & Skills

  • Register Nurse (RN) licensed to practice in the State of Colorado.
  • Minimum of 2 years of clinical experience in skilled nursing, long-term care, or acute care settings.
  • Experience in admissions, case management, or utilization review preferred.
  • Strong understanding of PDPM, ICD-10 coding, payer sources, and reimbursement methodologies.
  • Knowledge of federal and state regulations, including CMS guidelines and Medicaid/Medicare policies.
  • Exceptional critical thinking and clinical judgment skills, with the ability to assess and determine appropriate levels of care.
  • Meticulous attention to detail and the ability to effectively manage multiple tasks and priorities, ensuring all aspects of the admissions process are completed accurately and in a timely manner.
  • Proficiency in Electronic Health Records (EHR) and clinical documentation systems.
  • Strong organizational, time management, and communication skills, with the ability to work cross-functionally with clinical and administrative teams.

      Compensation and Benefits

      • Pay Range: $76,200.00-$97,100.00 per year

        • Full-Time (32 hours/week) Team Members are eligible for all benefits listed below.
        • Part-Time Team Members are eligible for select benefits.
