The Oncology Case Manager, in partnership with physicians, nursing, and healthcare team members, utilizes professional medical social work skills to assess patient and family needs for information, resources and referrals, psychosocial and counseling support needs, and establishment of plans for effective management of the identified needs. Assessment, planning, intervening and reassessment are performed in a collaborative environment. The Oncology Social Work Case Manager includes proactive, individualized planning for patients' progress across the continuum that optimizes quality of care, patient satisfaction, utilization, and cost.
Minimum Knowledge, Skills, & Abilities Required :
A. Master's prepared Social Worker from a program accredited by the Council of Social Work Education with a current Maryland licensure of LCSW or LCSW-C.
B. Oncology Social Work certification is preferred but not required.
C. Three to five years of work experience in a health care setting preferred.
D. Prior experience Case Management, Oncology, and / or Hospice experience preferred.
E. Prior experience with clinical pathways desirable.
F. Demonstrates effective interpersonal and communication skills.
G. Demonstrates tact, diplomacy, negotiation skills, and customer relations.
H. Ability to apply creative problem solving skills.
I. Ability to prioritize assignments and effective time-management skills.
J. Knowledge of diagnosis, expected treatment, and available resources
K. Basic knowledge of clinical and psychosocial aspects of patient care.
L. Ability to present a professional presence and appearance.
M. Must be detail-oriented, flexible, and committed to patient advocacy.
N. Ability to work independently and interdependently.
O. Demonstrates clinical skills in planning, organizing, and managing multiple functions and complex processes.
P. Knowledge of basic computer software programs.

Pay is based on experience, skills and education. If position is part-time, salary will be pro-rated based on scheduled hours. The pay range may also vary within the stated range based on specialty if applicable. Non-Exempt positions may have shift differential and / or Overtime paid, if applicable.
Salary pay range : $63,500 - $91,873.60
Hours : Monday-Friday, 8 : 00AM-4 : 30PM
