Frederick Living, part of Garden Spot Communities, is located in Zieglerville, PA (Montgomery County). We are a Continuing Care Retirement Community (CCRC) offering a full spectrum of care from retirement living, personal care, licensed healthcare, rehabilitation services, respite to memory care services.

Status: Full Time Day
Schedule: Monday - Friday 8am - 4:30pm
Salary Range: $28.00 per hour

As a Social Worker, supporting our mission will include the following responsibilities:
- Provides staff training regarding the psychosocial needs of residents.
- Provides follow-up to resident's post-hospitalization or transfers from/to another level of care; assesses adjustment to the new living environment, assesses transfer needs or level of care changes with residents, staff, and family. Arranges support services.
- Serves in a problem-solving capacity, as an advocate for residents. Provides accurate assessments and intervenes with effective treatment plans that empower residents' coping skills.
- Maintains a written record of the frequency and nature of any qualified social work consultations and services provided or obtained.
- Counsels and provides crisis intervention with residents and/or families as needed.
- Assists residents and/or family with financial, healthcare provider or insurance questions.
- Makes referrals to homecare, hospice, Office of Aging, counselors, psychiatrists, psychologists and/or others as needed.
- Acts as a resource to other staff on the psychosocial needs and problems of residents and their family members.
- Orients new residents to Frederick Living services and assists in their adjustment to the routines and schedules.
What do I need for this role with Frederick Living?
- An understanding of and willingness to live out our mission and values, providing compassionate person-centered care to all residents
- College graduate with a minimum of a BA from a school of social work accredited by the Council on Social Work Education. Experience with geriatrics is preferred.
- Fundamental knowledge of social service programs available to elderly is a benefit to have, and/or willingness to be trained in specific programs utilized in the department.
- Ability and desire to work with the elderly; ability to build relationships.
- Good verbal and written communication skills.
- Proficient computer skills.
- Strong organizational techniques.
- Strong conflict-resolution skills.
- Compassion and understanding when dealing with challenging circumstances.
- Ability to convey a professional image when relating to residents and their families.
