We are a Continuing Care Retirement Community (CCRC) offering a full spectrum of care from retirement living, personal care, licensed healthcare, rehabilitation services, respite to memory care services.

As a Wellness Nurse supporting our mission will include the following responsibilities:
- Interview and assess new residents as needed and establish medical records and other pertinent data
- Assist with completing yearly Support Assessments and report significant changes in resident's status
- Assess residents changing needs and reports to management
- Assist with coordination of residents' laboratory orders and results
- Assist with the integration of services such as: medical, emotional, psychological, rehabilitation, home care and social services
- Communicate with residents, families, staff, service providers and physicians, as needed, to assist residents in maintaining independence
- Maintain medical history on residents. Update changes and maintain residents' charts in good order
- Instruct residents regarding healthy living and disease prevention
- Respond to Residential Living emergencies and coordinate needed interventions
- Assist with annual influenza immunizations
- Assist with employee health functions such as influenza immunizations, annual PPD, etc.
- Abide by the organization's code of conduct in the performance of their duties
- Perform other related duties as required

- Licensed Practical Nurse. Experience with geriatric population preferred
