Free Life Wellness Center is a licensed, outpatient Community and Behavioral Health Agency conveniently located in Mesa, AZ. Our talented and dedicated team of clinicians and staff are trained to provide innovative and effective home and outpatient services to assist in the support and preservation of families.

Essential Job Functions

· Complete case management assessments.

· Conduct comprehensive assessments of clients' needs and circumstances.

Develop, implement, and monitor individualized service plans that address client goals.

· Provide ongoing supportive and/or case management functions in accordance with the problems, needs, and the strategies identified within the case management plan to assist the clients in achieving the stated goals and objectives.

· Maintain communication and collaboration with community resources.

· Provide ongoing face-to-face reviews with clients to assess the progress made in reaching service goals so that the service plan can be modified as necessary to ensure that the goals and objectives are being achieved.

· Document all service contacts including face-to-face, family communication, and updates. Demonstrate appropriate & complete documentation of treatment-related information including case management notes, progress notes, correspondence, releases of information, etc.

· Facilitate case management groups.

Facilitating CFT meetings (establishing an agenda, setting and maintaining a strength based meeting environment, staying within timeframes, giving all team participants the opportunity to contribute, holding people accountable for their commitments and the ability to assist a team in developing a consensus) for children and families.

· Participate in regular interdisciplinary staff meetings.

· Maintain collaborative relationships and communication with community referring agencies to formulate appropriate collaborative intervention strategies.

· Communication with client families to promote solidarity of client care and answer all questions/address family concerns.

· Implementation and education of life skills with clients individually. Life skills criteria include money management, time management, medication management, goal setting and planning, communication, and interpersonal skills, and decision-making and problem-solving.

· Works collaboratively in a team environment.

· Participates with program management team and other organizational teams or committees as appropriate and requested.

· Attends and participates in all required meetings.

· Performs work and represents the organization in accordance with our mission, vision, and values.

· Maintains consistent attendance and functionality to provide consistency in serving clients.

· Performs other duties as assigned, which are consistent with the general responsibilities of the position and in compliance with policies and procedures.

May be required to transport clients to appointments and program related activities.

Minimum Education and Experience

Preferred: Associates and/or Bachelors’ Degree

Preferred: Experience working with behavioral health children populations.

Preferred: 2 years of experience in a similar role.

CPR/ First Aid certification

Must have a Valid Driver’s License and Auto Liability Insurance

Must have an active Level 1 Fingerprint Clearance Card

Must be at least 21 years of age

May require off-site work and a reliable vehicle is required

Job Type: Full-time

Pay: $21.00 - $23.00 per hour

Expected hours: 40 per week
