What are the responsibilities and job description for the Case Management/Utilization Review/Discharge Planner position at Freedom Behavioral Hospital of Greenville?
Job Summary: Assesses, plans, implements, and evaluates the needs of patients. The LPN case manager is responsible to evaluate each admission to determine if InterQual criteria for severity of illness/intensity of service has been met. The case manager assigns provisional DRG’s, obtains and documents third party payer authorizations as well as participates in interdisciplinary discharge planning meetings. The LPN case manager facilitates timely, appropriate and cost-effective care through ongoing monitoring and communication with patient, physicians, and care givers.
Regulatory Requirements:
- Current Licensed Practical Nurse in the state of Mississippi
- At least 5 years of experience in clinical nursing
