Job Description

Title: Independent Living Advocate/Transition Coordinator

Reports to: MFP/ADRL Assistant Program Director

Employment Status: Nonexempt/Full Time Regular Staff

Position Summary: This position is based out of the Jamestown office and covers Stutsman and Barnes counties. Additional service area territory coverage based on consumer referrals. Its main focus is to provide services to empower consumers to meet their Independent Living goals.

Job Responsibilities:

Essential

  • Assist consumers set goals, develop strategies and make progress on the goals they have chosen.
  • Provide Nursing Facility Transition services to MFP eligible people, and back-up services for other Transition staff.
  • Assist consumers with planning and coordinating for a successful transition to community living.
  • Coordinate payment of one-time moving costs and rental assistance for consumers transitioning to community living
  • Maintain accurate records, receipts, and documentation for all transitions.
  • Advocate for consumers.
  • Teach independent living skills to consumers.
  • Provide independent living skills assessments and training.
  • Provide information, referral, and assistance services.
  • Provide referrals for other Freedom services, as appropriate.
  • Perform outreach to target population and referral sources.
  • Maintain knowledge and awareness of current legislative and regulatory developments related to disability services and issues.
  • Maintain records and forms, including service provision documentation for organizational reports.
  • Attend staff meetings and training as scheduled.

Secondary

  • Identify and participate in community groups as assigned.
  • Assist Freedom Resource Center staff in the provision of and systems advocacy and community education as directed.
  • Assist with special events.
  • Perform other duties as assigned by your supervisor.

Qualifications:

  • Personal experience with a disability is preferred.
  • Bachelor’s degree from a four-year college or university; or have four years of related experience and/or training
  • Must be motivated, self-directed and able to work independently
  • Ability to work as a team member.
  • Knowledge of community resources
  • Computer literacy
  • Possess and demonstrate knowledge of independent living philosophy, disability rights legislation, human service systems commonly used by people with disabilities, and the medical, social, and psychological aspects of disability
  • Interact with consumers in a professional manner with respect for their rights
  • Ability to verbalize and express ideas clearly in written, oral, and computer communications
  • The ability to advocate on issues promoting the independence of people with disabilities
  • Reliable means of transportation

Specific Equipment Used: Telephone, computer, personal automobile.

Physical Effort: While performing the duties of the job, the employee is usually seated, standing, or walking at will or meeting with consumers in a variety of settings. Limited physical effort is required. Regular and recurring travel required.

Salary Range: $21.00-$23.00 hourly, depending on education and experience.

Job Type: Full-time

Pay: $21.00 - $23.00 per hour

Expected hours: 40 per week
