What are the responsibilities and job description for the HHA/CNA (On Site Home Health) PT position at Freedom Senior Management?

This is an opportunity for you to do 'agency type' work, right here on our campus! No traveling to client homes!Schedule : 8am - 8pm and 8pm - 8am shifts; Must be willing to work 4 hr, 8 hr or 12 hr shifts; Every other weekend and holidays are requiredJob Duties :
- Provide direct care services under the direction of the Registered Nurse, including assistance with bathing, feeding, dressing / undressing, toileting, ambulating, transferring, vitals, medication reminders, etc.
- Perform household services essential to health care at home, including but not limited to meal preparation / feeding, laundry, light housekeeping, grocery shopping and transportation.
- Completes the appropriate records to document care given and pertinent observations.
- Communicate effectively with all members of the interdisciplinary team through verbal reports, participation in staff meeting, and team conferences, as requested.Note : travel is negligible but must be willing to drive residents, as neededCertified Nursing Assistants and Certified Home Health Aides are encouraged to apply!
