Our campus including Independent Living, Assisted Living and Skilled Nursing, has been completely remodeled. We are looking for a Health Services Director to complete our Leadership Team in Independent Living. The Health Services Director is a licensed nurse and oversees the health and wellness of our residents in Independent Living.
Duties include:
Works closely with the Executive Director to ensure each resident in Independent living is getting the care and support they need to thrive.
Identifies residents who need a higher level of care.
Coordinates the total care of each resident as needed, which comply with physician’s orders, regulations and Freedom Village policies.
Supervises wellness clinic/nursing and EMS staff scheduled from 4pm-8am. Lead EMS reports to this position.
Adhering to all company and regulatory (including Title 22) standards. Ensure all department policies are updated to meet regulatory requirements.
Perform pre-appraisal and MOCA assessments of potential residents to determine the care support needed and appropriateness for Independent Living.
Continually assess our existing residents to make sure their needs are being met. Identify residents that need more support and work with appropriate departments to provide support.
When residents are in the hospital, communicate with Case Manager/Discharge Planner to determine level of care needed for resident upon return. Coordinate with appropriate departments to ensure care is in place upon discharge.
Implements /suggests medication management for residents who need or will benefit from this support.
Annually updates medical information sheet for each resident.
Ensures nurses, EMS personnel properly fill/pass medications in accordance with their license/certificate and regulations.
Performs annual re-appraisal assessments for residents with dementia.
Coordinate with the Health Center team to get updates on residents temporarily in the Health Center to best support their return to Independent Living and assist in coordinating services to enhance their independence. Communicate regularly with our exclusive outside caregiving agency and internal departments on the care and support provided for our residents.
Ensure continuity of health services and care for residents across nurses and EMS staff.
Implement and maintain an accurate wellness record for each resident.
Maintain incident reports, pet policy vaccinations and rental insurance, and scooter/electric wheelchair required information for residents.
Supervise the storage, distribution, administering of and discontinuation of resident medication.
Serve as the liaison between residents and their families, administration and staff as well as with the resident physician, pharmacist, and medical personnel in regard to the resident’s health.
Submits required state reports to the Department of Social Services within the timelines required.
Current LVN license required. RCFE preferred and required within first 6 months of employment.
5 years of experience working in Senior Living required.
Previous Director/Manager level experience in Senior Living strongly preferred.
