What You'll Do
Fulfill five-year Primary Care Training and Enhancement – Residency Training in Mental and Behavioral Health (PCTE-RTMB) grant project goals and objectives by participating in a multi-disciplinary health care team in service delivery for patients at the Continuity of Care Clinic. Responsible for coordinating care for all patients and assuring communication between the clinical team, patients, and their families. This position will provide Health & Behavior & Psychotherapy Assessments/Intervention at the Freeman Neosho Academic Family Medicine Clinic to empower patients of all ages to develop healthy habits and improve well-being. Supervision will be provided for MSW, by an LCSW.
Schedule: Full Time Days (40hrs/week)
Requirements
Current Missouri licensure as a Licensed Clinical Social Worker OR Masters in Social Work working on LCSW.
Preferred Requirements
Master's Degree in Social Work
Work experience with behavioral health social work preferred.
Work experience with critical care environments strongly preferred.
